Appointment Information

Information to bring to an office visit:

Please bring your picture ID, insurance cards, list of medications you are currently taking with dosage, any medical records, test results or films to your visit with us. If your insurance requires a referral, please come prepared with your Primary Care Physician Referral and be prepared to pay your co-payment as required by your insurance company.

If you are a new patient to our office, please arrive 30 minutes early to complete the paperwork. Or, for you convenience, you may download your forms from our website and fill them out to bring with you for your appointment.

Insurance

Most major insurance plans are accepted. Please call our office for details. Please call (601)296-3070 Ext 5003 for any billing questions.

Our office hours are 8:00am to 5:00pm, Monday through Friday with the exception of major holidays. During normal office hours our staff is available for scheduling appointments, clinical inquiries and prescription refills.
